It happens. When Sly & the Family Stone's album "Fresh" was released on CD, they pressed and distributed the wrong mix of the album to stores. It's way, way different than the LP and the correct CD version that came out later. The CD was only in stores for a day or two before the label realized what happened, and they recalled them all. Of course every record store in the world said, "Uh, we, uh.... sold them all already. Yeah." and promptly jacked the price up or bought them for themselves.
You'd be amazed at how little people at major labels know about their tape vaults.
